The Massachusetts Department of Children and Families has fired a third person in connection with the disappearance of 5 year old Fitchburg Boy Jeremiah Oliver.  The Department had been supervising the Oliver family since September 2011, and today released a final report on its failings in the case.

Jeremiah Oliver has been missing for 3 months and is presumed dead. DCF has already fired a social worker and a supervisor and it has now fired a manager and disciplined another one.

“It is my belief that the failure and misconduct of the staff involved in this case contributed to this tragedy,” says DCF Commissioner Olga Roche.

A final report by the department places most of the blame on the social worker who was required to visit the 5 year old and his two siblings every month. But the last time she saw them was in April, even though she continued to receive call after call that the children were being abused.

Roche says the manager and the Supervisor knew that the social worker was failing to make home visits but didn’t do anything.

“The supervisor entered false information, indicated that the children were taken care of, that they were doing well, as well as that the home was well furnished and there was plenty of food in the home,” says Roche. “This information was never available to them because they never visited the home.”

Roche says she believes the Oliver case is unique and is directing any future questions to law enforcement.
